Facilities shall obtain and keep in force liability insurance.
Proof of Professional and General Liability insurance including worker's compensation insurance must be submitted at the time of application. Liability insurance must cover legal liability for death, injury, or disability of any human being, or for damage of property, with provision for medical, hospital and surgical benefits to the injured person, irrespective of the legal disability of the insured, when issued as a part of the liability insurance contract.
