Section V-5115 - Case Closure
A. Cases are closed by CSHS only after it has been clearly established that at least one of the following reasons exist. The following are closure codes and corresponding reasons.

Definite diagnosis not established within one (1) year

Further treatment not indicated

Family no longer interested

No CSHS treatment available

Over age

Under care of another agency

Refusal to sign necessary required documents (i.e., subrogation, insurance, etc.) and/or make application to other funding sources (i.e., SSI, Medical Needy, Medicaid, etc.)

Moved from the State of Louisiana

Death

Other

The "Other" category is used only after thorough assessment and intervention.

Examples of situations falling in the "Other" category are: a. Excessive (3) missed appointments b. Non-compliance with treatment plan to the extent that the patient's progress is impeded (CSHS Central Office disposition is required prior to closure.)
