Section V-5115 - Case Closure

A. Cases are closed by CSHS only after it has been clearly established that at least one of the following reasons exist. The following are closure codes and corresponding reasons.

Code	Reasons
01	Financial ineligibility/unable to establish eligibility
02	Definite diagnosis not established within one (1) year
03	Further treatment not indicated
04	Family no longer interested
05	No CSHS treatment available
06	No treatment indicated
07	Over age
08	Under care of another agency
09	Under private care
10	Refusal to sign necessary required documents (i.e., subrogation, insurance, etc.) and/or make application to other funding sources (i.e., SSI, Medical Needy, Medicaid, etc.)
11	Moved from the State of Louisiana
12	Death
13	Other
The "Other" category is used only after thorough assessment and intervention. Examples of situations falling in the "Other" category are: a. Excessive (3) missed appointments b. Non-compliance with treatment plan to the extent that the patient's progress is impeded (CSHS Central Office disposition is required prior to closure.)
