Section 410 IAC 6-7.2-20 - Records

Sec. 20.

(a) A record for each camper must be maintained by the designated adult operating a camp and shall contain the following:
(1) The camper's name and address.
(2) The name, address, and telephone number of the camper's parent, legal guardian, or designated adult emergency contact.
(3) Authorization from the parent or guardian for emergency medical care.
(4) A list of relevant health conditions that camp personnel may encounter.
(b) Records required by this rule shall be kept on file by the designated adult for a period of at least two (2) years.
