Section 410 IAC 32-3-7 - Expiration of course approval; reapproval

Sec. 7.

(a) Unless reapproved, a training course approval, including refresher training approval, shall expire thirty-six (36) months from the date of issuance. A training course provider seeking reapproval of each training course shall submit one (1) written application per discipline for each initial and each refresher training course on forms available from the department no later than ninety (90) days before its current approval expires. The department cannot guarantee that a determination on the application will be made before the end of the current approval period if a training course provider does not submit a timely, complete application for reapproval.
(b) The training course provider's application for reapproval shall contain the following information:
(1) A completed and signed application form for lead-based paint training courses.
(2) The names and qualifications of the course instructors, including guest instructors and academic credentials and field experience.
(3) A description of any changes to the training facility, equipment, course materials, or curriculum since its last application was approved that adversely affects the students' ability to learn.
(4) A statement signed by the program manager stating that the training course provider complies at all times with:
(A) all applicable requirements in this rule; and
(B) the record keeping and reporting requirements of this section.
(c) Upon request, the training course provider shall allow the department to audit the training curriculum to verify the contents of the application for reapproval.
(d) A training course provider may apply for reapproval to offer initial courses or refresher courses in as many disciplines as it chooses or the Indiana lead-based paint rule awareness course. A training course provider may seek approval for additional courses at any time as long as the training course provider can demonstrate that it meets the requirements of this rule.
