Section 410 IAC 29-3-2 - Reporting of case information

Sec. 2.

(a) Local health officers shall ensure that case information is reported to the department for children less than seven (7) years of age who have an elevated blood lead level.
(b) Case management activities shall be reported electronically using the forms designated by the department.
(c) Case closure activities shall be reported electronically using the forms designated by the department.
