Sec. 6.
(a) In order to renew its abortion clinic license, the clinic shall file an application for the renewal of an abortion clinic license with the division at least one (1) month before the expiration of the current license. (b) The renewal application includes the following:(1) An application for the renewal of a license to operate an abortion clinic on a form prescribed by the division to include the selection of only one (1) of the following procedure classifications: (A) Surgical abortions only. The clinic is precluded from performing drug induced abortions.(B) Both drug induced abortions and surgical abortions.
