ORDER APPROVING FORM OF LEGGETT & PLATT DIRECT PURCHASER SETTLEMENT AND THE CARPENTER DEFENDANTS DIRECT PURCHASER SETTLEMENT CLAIM FORM

1. This Court has preliminarily approved Direct Purchaser Class Plaintiffs' ("DPPs") settlement agreements with Defendants Leggett & Platt, Incorporated ("L&P") and Carpenter Co., E. R. Carpenter, L.P., and Carpenter Holdings, Inc. (collectively, the "Carpenter Defendants") (Docs. 1391 & 1406).

2. This Court has approved DPPs' Notices of Class Certification and Settlements, and set a deadline of November 24, 2014 for mailing Notices to the Direct Purchaser Class (Docs. 1397 & 1406).

3. Court approved Notices indicate that they will include a claim form for potential Claimants.

4. DPPs, with input from L&P and the Carpenter Defendants, have generated a proposed claim form for circulation with the Notices of the L&P and the Carpenter Defendants Settlements, which is entitled "Leggett & Platt Direct Purchaser Settlement and the Carpenter Defendants Direct Purchaser Settlement Claim Form" ("Claim Form"), attached as Exhibit A.

5. The Claim Form is substantially similar to the Vitafoam Settlement claim form approved by this Court (Doc. 457).

6. Much of the updated language in the Claim Form was copied from the Long Form Notice approved by this Court, as well as the Leggett & Platt Direct Purchaser Settlement Agreement and the Carpenter Defendants Direct Purchaser Settlement Agreement (Docs. 1379, 1400 & 1406).

7. Therefore, this Court approves the Claim Form attached as Exhibit A for circulation with the mailed Notices of Settlement.
